Job Description: Reporting to the Manager of Chronic Kidney Disease - Renal Services and Home Based Therapies, the Patient Educator responsibilities will include: coordinate and participate in patient educator for chronic kidney disease patients requiring peritoneal dialysis and home hemodialysis. This will include liaison with families, physicians, dietitians, staff nurses, and community agencies. Provide individual outpatient education for patients and families in a clinic setting. Coordinate and participate in education programs for designated nursing staff that will be providing peritoneal dialysis for patients at SPH and other agencies. Serve as a primary clinical resource for nursing, medical and other professional/support staff. Participate in departmental activities (program development, planning, quality improvement, and evaluation). Participate in research and evaluation projects.

Human Resources Exemption: No

Experience

  • Experience with Peritoneal Dialysis and Hemodialysis is preferred
  • 3 years related clinical experience with chronic kidney disease

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Nursing diploma is acceptable
  • Master of Nursing, Adult or Continuing Education is an asset
  • Basic Life Support Certificate (BLS)
  • CNA certificate in clinical practice area is an asset

Competencies

  • Communication skills to transfer knowledge/skills
  • Advanced - Assessment skills

Knowledge and Abilities

  • Ability to competently use computers and software applications
  • Ability to work independently and as a member of a multi-disciplinary team
