Job Description: Reporting to the Care Team Manager the Resident Care Coordinator is responsible for supporting the highest standard of care and service provision to residents living on a given neighborhood. As an experienced practitioner, the resident care coordinator acts in a leadership role to provide specialized clinical expertise, coordination of resident care, and provide support and teaching to the resident care team (including residents and residents families) within a collaborative environment. The routine procedures are generally predetermined but critical thinking and independent judgment is an integral part of the position.

Experience

  • Three (3) years nursing experience

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Basic Life Support Certificate (BLS)
  • Nursing diploma is acceptable

Competencies

  • Basic - Writing skills
  • Leadership skills
  • Verbal communication skills
