Job Description

The Saskatchewan Health Authority (SHA) - The Bridge Clinic is a temporary medical home, designed to meet the needs of patients who require medical care and do not have a family doctor in the community. We are looking to contract 56 hours of physician services per week, following a 7 day a week service delivery model. Individual physicians will provide 4-5 hours of clinical service evenings and weekends.

We anticipate multiple physician contracts and hope to enhance the current family physician capacity within the city. The Bridge Clinic will operate in evenings and weekends with preference give to family physicians who concurrently practice in longitudinal care.

The Bridge Clinic physician(s) will work as part of the team consisting of nurse practitioners, primary health care registered nurses (RNs), and assessor/coordinators. The physicians will assess patients as part of a coordinated appointment with the primary health care RN, and the nurse practitioners will function as the primary care practitioner. The nurse practitioners will be responsible for ongoing follow-up care with physician support as needed. All team members are expected to participate in multi-disciplinary team based care and consultation. Using a client centric approach, patients of the clinic will have access to Primary Health Care services such as chronic disease management, central therapies, pharmacy, mental health & addictions, wound care. Assessor/coordinators & nurses will provide system navigation to health resources in the community. Referrals will be from acute care in phase one with potential expansion as capacity permits.

Location of practice:
Saskatoon, Saskatchewan - The Bridge clinics will deliver services in leased spaces within existing clinical settings across the city in order to provide services within client local community where possible.

Job Qualifications

Successful applicants will be eligible for a provisional or regular license with the College of Physicians and Surgeons of Saskatchewan (CPSS) Candidates must obtain privileges with the SHA to practice family medicine in Saskatoon.
