Job Description: The Primary Health Care Case Manager serves as a single point of access to an integrated health care team, participates in collaboration with the patient, in identifying health needs and coordinates resources toward meeting those needs.

Education

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • Equivalent combination of education and experience is acceptable

Competencies

  • Intermediate - Computer skills

Knowledge and Abilities

  • Ability to work under pressure and establish priorities
  • Ability to initiate and exercise critical thinking and decision-making
  • Ability to delegate appropriately.
  • Facilitation, consultation, listening and other relationship-building skills
