This posting is for 1(one) permanent part-time position working 60 hours bi-weekly.

This posting is anticipatory full time permanent positions working 75 hours bi-weekly in-Home Care - Continuing Care.

It will establish a 12-month eligibility list which may be used to fill similar positions.

This job is based in the thriving and diverse city of Whitehorse in the beautiful Yukon.

Your role

The Yukon Home Care Program provides client-focused health services in the home, improving quality of life and supporting Yukoners to live independently. Home Care offers a range of services tailored to meet individual needs, including support for acute care, chronic and palliative conditions. These services are determined through standardized assessments to assist in creating individualized plans of care; and are supported by an interdisciplinary team, consisting of Physiotherapists, Occupational Therapists, Licensed Practical Nurses and Home Support Workers.

As an integral team member, the Registered Nurse service delivery includes; client specific care coordination, liaison with the medical community direct nursing care, such as; education, treatment related to medical conditions (i.e. wound care, ostomy care etc.), collaboration with a multidisciplinary team, development of care plans, funding requests, and other nursing duties.

Essential qualifications

  • Eligible for Registered Nurse registration with a Canadian registered nursing regulatory authority.
  • Recently applied knowledge of nursing theory, practices & techniques.

  • Ability to provide professional care for residents by promoting their physical, emotional and spiritual well-being.
  • Ability to provide leadership, coaching and training to members of the team.
  • Effective oral and written communication skills.
  • Excellent organizational skills.
  • Experience working in a community care setting.
  • Excellent critical thinking, relationship building and conflict management skills.
  • Ability to operate across multiple levels of care and within a transdisciplinary environment.
  • Strong understanding of working with various cultural backgrounds

Conditions of employment

You must meet the following conditions before being hired, or within the time frame noted.

  • TB screening
  • Vulnerable Sector RCMP security clearance
  • CPR Level C with AED (Home Care) (completed within last 30months)
  • Transportation of Dangerous Goods (complete within last 30 months)
  • WHMIS 2015
  • Valid class 5 Driver's License without driving restrictions
  • Current registration with the Yukon Registered Nurses Association.

Job requirements

This job requires you to perform, and/or be exposed to, the following:

  • Shift work (days, evenings, weekends, including stat holidays),
  • Exposure to plants and/or flowers, animals, latex products, second-hand smoke from residents/clients
  • Heavy lifting 50 lbs and over, bending.
  • Travel during inclement weather conditions may be required.
